You can use this quick form to make a reimbursement claim for your dental, optical and private GP costs or for claiming your maternity cash benefit. Please have your policy number and treatment invoices ready.

Please make sure your treatment is covered by your policy by checking your policy certificate. Please also note that a policy limit and excess might apply to your Dental Costs claim.

What's covered under the Dental Costs

  • Routine - e.g. check-ups, x-rays and hygienist fees
  • Accident - treatment of an accidental dental injury that occurred after the start date of your cover with us (only available to Freedom Elite clients)

What's not covered under the Dental Costs

Some examples of what’s not covered: Veneers; Implants; Toothpaste; Mouthwash; Prescriptions; Medication; Braces; Retainers; Mouth guards, Dentures.

If you are claiming for more than one person, please submit a separate claim for each person.

Some examples of what’s covered: Eye tests; New prescription glasses or sunglasses; Contact lenses (in bulk).

What's not covered under the Routine Optical Costs

Some examples of what’s not covered: New glasses or sunglasses with no change in prescription; Contact lenses schemes; Glasses cases; Contact lens solutions.

Some examples of what’s covered: Consultations; Tests; Minor surgeries carried out in the GP’s own surgery.

What's not covered under the Private GP Costs

Some examples of what’s not covered: Administration charges; Home visits; Prescriptions; Medication; Dressings.

Please note that the Maternity Cash Benefit covers any children born after the start date of your policy. It won't cover children born before you took out the policy. A 10-month qualifying period applies to this benefit which means your policy has to be at least 10 months old to qualify for this benefit. The maternity cash benefit is £150 per child.
